• Access your Machine Readable Files - The files are a specific file type, JavaScript Object Notation (.json.gz file format) as prescribed by the law. Please note, as machine-readable files, they are not readily interpretable by consumers. As such, members are encouraged to use the more member friendly Find Costs tool available through myWellmark.com. The tool can help you find providers and even determine how much a visit will cost you before you receive services. The machine-readable files do not provide information on a member’s out-of-pocket charges.

Members can lookup to see if their doctor is in-network by going to the Provider Finder tool via https://www.humana.com/finder/ and using the attached PDF to walk them through the process. They will want to base their search off of the Medicare PPO/Employer PPO Plus network option. They will also want to select the coverage type “Medicare or Medicare – Medicaid” and select the 2021 plan year.

To help you easily understand your health plan benefits, a Summary of Benefits and Coverage for your current health plan can be viewed here on the City’s website. The SBC contain a full description of your health plan and benefits. It is designed to provide simple and consistent information using terminology and a format that has been standardized among all health insurance companies.

The SBC summarizes the key features of your policy such as a description of the coverage, deductible amounts, cost-sharing obligations and coverage limitations and exceptions.
